thanks for putting this up. just one possible typo:

in step 2 you have

....
 old_passwords=1

log-bin
binlog-do-db=<database name>  # input the database which should be replicated
....

shouldn't log-bin be followed with a filename like ?

log-bin    =   /var/log/mysql/mysql-bin.log
